Labelling Requirements

Labelling and Shipping Your Sample

To help your sample reach us safely and avoid delays, please follow these labelling and shipping steps carefully:

  • Complete all forms
    Include all requisition forms provided with your kit. We cannot process your sample without them.

  • Label your samples correctly
    Ensure every sample tube and piece of paperwork is clearly labelled with:

    • Full name

    • Date of birth

    • Date the sample was taken

Clients are responsible for labelling their samples accurately. Check your kit instructions for any additional labelling recommendations.

  • Use the provided bags and packaging
    Place your sample in the biohazard or ziplock bag supplied, with the absorbent material included. Pack the sample securely in the original kit box (or another sturdy box if needed).

  • Attach the shipping label correctly
    Keep the DHL bag flat and place the pre-paid return label in the plastic sleeve on the back side only. Do not stick the label directly on the bag.

  • Seal and prepare for return
    Place your boxed sample inside the DHL bag and seal it carefully. Keep the bag flat and spacious to avoid covering any markings.

Things to Avoid

  • Do not ship on a Friday or the day before a UK bank holiday

  • Do not place the DHL label inside the box or bag

  • Do not send your sample without a box

  • Do not place forms loose inside the bag or in the outer sleeve

  • Do not cover any markings, including:

    • UN3373 diamond

    • Heart symbol

    • Pink line with TDX wording

    • Biological Substance Category B wording

    • OVERPACK

    • MEDICAL EXPRESS

    • DHL EXPRESS

Important: If the label covers any of the above, DHL may reject your package and return it to you.

✅ Labelling & Shipping Checklist

Before returning your sample, confirm:

  • ✅ All requisition forms completed and included

  • ✅ Sample tubes and paperwork labelled with full name, DOB, collection date

  • ✅ Sample placed in biohazard/ziplock bag with absorbent material

  • ✅ Sample securely packed in original or sturdy box

  • ✅ DHL label placed in plastic sleeve on back of shipping bag

  • ✅ Bag sealed flat, without covering any markings

  • ✅ Shipment scheduled Mon–Thu for UK returns
